Non-Diagnostic / Keepsake Disclaimer
South Florida 3D Ultrasound provides elective, non-diagnostic ultrasound sessions for bonding and keepsake purposes only. Our sessions are considered "entertainment" keepsakes. We do not diagnose medical conditions, make statements about fetal health, or provide medical advice. Images obtained may vary based on baby's position and stage of pregnancy.
Not a Substitute for Prenatal Care
Our elective ultrasound is not a replacement for the diagnostic ultrasounds and routine prenatal care provided by your physician or midwife. You must remain under the care of your OB or midwife throughout your pregnancy. Supervising physician: [Supervising physician name & office - to be provided].
Informed Consent & Waiver
Prior to your session you will be asked to review and sign an informed consent and liability waiver, including confirmation that you are under the care of a licensed prenatal provider. If anything of concern is observed during a session, we will advise you to follow up with your healthcare provider; we do not provide a diagnosis.
Privacy
When you submit an appointment request we collect your name, contact details, and appointment preferences solely to schedule and communicate with you about your visit. We do not sell your information. To request deletion of your information, email [email protected].
